Submit a resident request
This form collects the resident's story and music preferences so we can brief the artist properly.
Before you start
- ✓Confirm you have consent from the resident or their family/representative.
- ✓If you're unsure about any details, it's fine to write “Not sure”.
Feedback form
Log a session (60 seconds)
After you've played the song, log the resident's response. This helps us improve and makes future songs even more personal.
